Objectives: Intertrochanteric femur fractures are prevalent injuries among the elderly, significantly affecting their quality of life (QOL) and functional status. This study aimed to evaluate functional outcomes and QOL in elderly patients one year after sustaining an intertrochanteric femur fracture.Methods: This retrospective observational study was conducted at Shahid Beheshti Hospital in Kashan, Iran. The functional status and QOL of 79 elderly patients with intertrochanteric femur fractures, who were at least one year post-injury were assessed between February 2020 to February 2021.Data were collected from patient’s records, which included sociodemographic and clinical information at the time of admission. Functional status was evaluated using the Barthel Index, and QOL was assessed using the shortened form of the SF-36 questionnaire.Results: The study population comprised 60.8% women, with a mean age of 79.81±7.07 years. The mean of the Barthel index score was 11.49±2.22, indicating that participants achieved approximately 60% of the maximum functional score. The mean overall QOL score was 50.59±9.54, suggesting that patients attained approximately 51% of the maximum QOL score. A final linear regression model indicated that increased age was significantly associated with declines in both functional abilities (R²=0.53, p<0.001) and QOL (R²=0.39, p<0.001). Additionally, patients who received physiotherapy demonstrated significantly better functional outcomes than those who did not.Conclusion: This study highlighted the significant impact of intertrochanteric femur fractures on the functional status and QOL of older adults. The findings emphasized the critical role of rehabilitation services, such as physiotherapy, in improving patient outcomes. Further research is warranted to explore the influence of comorbidities and optimize interventions for this vulnerable population.

Objective: To assess psychometric properties of the European Quality of Life 5-Dimension 3-Level Version(EQ-5D-3L) commonly used tool for measuring road traffic injury (RTI) patients’ quality of life.Methods: The psychometric study assessed the reliability and applicability of EQ-5D-3L through phonesurveys, based on a national cohort platform. Data of 150 RTI patients recruited from the cohort study wereincluded as 50 patients per each follow-up phase (one, six, and twelve months after discharge). A 12- day-timespan was between test and retest. We measured psychometric properties (internal consistency reliability andstability reliability) and agreement using Kappa coefficients and percentages of agreement and Bland-Altmanmethod. Data were analysed using software STATA statistical package.Results: The majority of patients were men (80%) with mean age (SD) of 41(14.7%), employed (78%) andeducated (86%). The Persian version represented high internal consistency reliability at total level (Cronbach’sα=0.81) and moderate to good reliability at phase levels (0.62-0.87). The stability reliability was excellent attotal (ICC=0.98, 95% CI: 0.97, 0.98) and phase levels (0.97-0.98. The kappa agreement coefficients were valuedmoderate to perfect (0.6-0.8, p>0.0001). The Bland-altman plot illustrated high agreement between test andretest scores. No floor and ceiling effects were found.Conclusion: The study revealed that EQ-5D-3L was highly reliable and responsive to be applied through phoneinterviews at three different times post injury and discharge, as no previous study considered its psychometricproperties at various phone follow-ups after RTIs.

Objective: To investigate the quality of life (QOL) of injured motorcyclists and associated factors in a period of three months after the accident. Methods: In the present study, we were included 190 injured motorcyclists who admitted to two referral specialized hospitals (Emam Reza and Shohada) in Tabriz, between June 2018 and January 2019. All injured motorcyclists were contacted through the telephone one and 171 of them (90%) three months after their accident to complete an EQ-5D-3L questionnaire. The baseline measurements were gathered by using face to face interviews in the hospitals. The QOL score could vary between 1 and 3. The higher score showed a lower QOL. Results: The injured motorcyclist’s QOL score was relatively better in three months after the accident (mean±Standard Deviation (SD): 1.78±0.51) in comparison with their status a month after the accident (2.15±0.65) (p <0.001). The multivariable model showed that individuals with pelvis injuries (Coef: 0.29, (95% CI: 0.16, 0.42), p=0.001) and knee injuries (Coef: 0.26, (95% CI: 0.10, 0.42), p=0.001), experienced a higher QOL score. Also, those whose accident had happened in rainy weather experienced higher QOL score (Coef: 0.33, (95% CI: 0.12, 0.53), p=0.001). The patients who were in an accident with a vehicle were experienced a better QOL than others (Coef: -0.26, (95% CI: -0.43, -0.09), p=0.002). Conclusion: The assessment of three-months post-accident showed that the QOL score of the motorcyclists was reduced. It is recommended that the QOL of patients should be improved in hospital discharge victims.

Objective: To evaluate the effects of dexmedetomidine in caudal epidural on controlling pain, erythrocyte sedimentation rate (ESR) and quality of life in patients with failed back surgery syndrome (FBSS).Methods: The study was a single-blind clinical trial. From the total of 70 patients suffering from low back pain caused by a failed back surgery syndrome were referred to Akhtar and Imam Hossein Hospitals between the ages of 25 to 75 years with a history of back pain more than 12 weeks and a visual analogue scale (VAS) score of higher than 3, and 50 people were randomly selected and divided into two groups of dexmedetomidine and control. The control group received an epidural dose of 10 cc containing triamcinolone and bupivacaine, and the dexmedetomidine group received an epidural dose of 10 cc, containing dexmedetomidine, triamcinolones and bupivacaine with diluted normal saline. Epidural caudal injections were performed in the abdomen in a laid down position. Before starting the study and at the end of the fourth week, the two test groups were measured for visual analogue scale (VAS) and ESR and were asked to complete the quality of life questionnaire.Results: Overall, 50 patients with FBSS were enrolled. The mean age was 53.88 ± 8.9 years (range 25–75); 54% (27/50) were men. The results showed that the injection of dexmedetomidine in epidural caudal was associated with decreased pain (p=0.001) and improved quality of life (p=0.022), while showed no significant effect on ESR (p=0.110).Conclusion: Administration of dexmedetomidine in the epidural caudal is effective in controlling pain and quality of life in patients with failed back surgery syndrome.Clinical Trial Registry: IRCT20181012041316N1
